Our MISSION is to provide access and assistance to older adults and individuals with disabilities in Dallas County.
Our VISION is to enhance the quality of life by empowering individuals with choices so they may achieve their full potential.
Working with key partnering agencies, Connect to Care hopes to provide streamlined self-directed access to information, resources, and services through the Internet complimented by a call center to assist those who require additional assistance.
The program has been modified to build on existing initiatives and integrated community-based services, in order to improve the flow of information. It is our intent to improve access to long-term services and supports for persons who are unaware of the options available.
Connect to Care is a collaborative community effort developed by an Advisory Board. The Advisory Board acknowledges that despite the existence of a wide variety of organizations that provide long-term services and supports in Dallas County, consumers continue to find it difficult and cumbersome to locate accurate information and obtain assistance they need to improve their lives. In which obtaining the right services in a timely manner are both complex and daunting. Multiple service agencies exist, but most have different avenues of entry and their own requirements. While there's an abundance of information on the Internet, finding the right information is not always easy.
The Advisory Board is made up of representatives from the following agencies:
211 Texas Dallas Region
2-1-1 Texas is a free, confidential, information and referral line answered by nationally certified specialists - 24 hours a day, seven days a week. When callers dial 2-1-1, they are connected to area information centers in their region. These trained experts have access to the most comprehensive database of community- based organizations, government agencies and nonprofits that exist in Texas. Information can be provided in almost any language.
Area Agency on Aging of Dallas County
The mission of the DAAA is to serve as the focal point and advocate on behalf of persons 60 years of age and older who reside in Dallas County by planning, developing and coordinating services that ensure a system that positively impacts their health, honor and dignity.
Texas Department of Aging and Disability Services
DADS strives to provide a comprehensive array of aging and disability services, supports and opportunities that are easily accessed in local communities. Our key responsibilities to the citizens of Texas include: Working in partnership with consumers, caregivers, service providers, and other stakeholders; Developing and improving service options that are responsive to individual needs and preferences; Ensuring and protecting self-determination, consumer rights, and safety.
Metrocare is a community mental health and mental retardation center serving the residents of Dallas. Governed by a nine-member Board of Trustees appointed by the Dallas County Commissioners Court, the Center provides a comprehensive array of services and support for people dealing with the challenges of mental illness and/or developmental disabilities.
REACH of Dallas
REACH OF DALLAS RESOURCE CENTER ON INDEPENDENT LIVING A project of REACH, Inc., a community-based nonprofit organization with the dual mission of providing services for people with disabilities so that they are able to lead self-directed lives and educating the general public about disability-related topics in order to promote a barrier free community.
The Alzheimer’s Association, Greater Dallas Chapter, provides support and assistance to persons affected by Alzheimer’s disease, their families and other caregivers. The Alzheimer’s Association encourages and supports research, improved diagnosis, prevention and cure.
The Arc of Dallas
The Arc of Dallas’s mission is to improve the quality of life for people with intellectual and related developmental disabilities. It was formed in 1951 by a group of parents who joined forces to fight for their children’s rights to be educated in the public school system. The Arc of today still serves as a voice in the local, state and national community. The Arc of Dallas service area includes Collin, Dallas and Rockwall counties. Most programs and services take place in local communities and neighborhoods in conjunction with community partners including schools, churches, recreation centers, and others.
Citizens Development Center
The mission of Citizens Development Center (CDC) is to empower adults with disabilities to achieve their highest level of employment. Citizens Development Center provides unique educational, training and employment assistance to individuals with disabilities to assist them in achieving their vocational goals and dreams. CDC operates two programs: Work Center and Employment Assistance. The Work Center Program provides intensive vocational support and structure to adults with severe disabilities who have been unable to secure or maintain employment or who choose to work in a protected environment. The Employment Program provides intensive, hands-on intervention to help individuals with disabilities secure and maintain community employment and reach lifelong employability - the capacity to hold rewarding jobs over one’s lifetime.
Special Care and Career Services
Special Care & Career Services supports families, children and adults with developmental disabilities through two age-determined programs: Early Childhood Intervention and Supported Employment Services. The program serves Collin, Dallas and Denton counties.
UCP is one of the nation’s leading organizations serving and advocating for more than 54 million Americans with disabilities. In north Texas UCP offers services to individuals, families and communities, such as early childhood development and education, community living assistance and support services (CLASS), home and community support (HCS), consumer direct services (CDS), group homes, adult Impact program, and assistive technology programs. Additionally, UCP provides occupational therapy, physical therapy, and speech therapy along with a vast array of adaptive equipment available through our equipment loan program.
