How can I find help for myself or someone else?
You can search for services that meet your needs by:

  • Using the Service Directory. The Service Directory contains information on State and local agencies that provide services to older adults and individuals with disabilities. You can search the directory by “Category”, “Program/Agency Name”, “Key Word”, or “Advanced Search”.
  • Submitting a “Self-Referral”. Complete the form, by filling in the required fields. Select the services you are currently receiving and the services you are requesting. Click “Submit”. The page will refresh with a list of agencies that meet your requirements. You will then be asked to select the agencies you want your information sent to (not all agencies in the Service Directory are part of the Connect to Care Community). If you select an agency that is not part of the Connect to Care Community, your form cannot be sent. A Connect to Care Systems Navigator will contact you and assist you with the referral.
  • Calling 1-888-743-1202, if you need additional assistance or feel uncomfortable submitting the information online, you can contact our toll-free number and speak with one of our Service Navigators. If an agency you choose is not part of the Connect to Care Community, the Service Navigator can conduct a warm transfer by staying on the line and connecting you to the preferred agency. Once the transfer has been made, the Connect to Care agent will disconnect.

How are the referrals handled?
Once your referral has been submitted to the designated agency, the agency will contact you within 5 business days.

What if I select an agency that is not part of the Connect to Care Community?
If you submit a “Self-Referral”, and choose an agency that is not part of the Connect to Care Community, you will receive a pop-up window informing you that your form cannot be sent and a Connect to Care Systems Navigator will contact you and assist you with the referral. Or, if you are speaking with a Connect to Care Service Navigator, he/she can conduct a warm transfer over the phone

What kinds of resources are available in the Service Directory?
The Service Directory contains information on State and local community agencies that provide services to older adults and individuals with disabilities. Such as-health care and childcare, job training, education and recreation, caregiver support, services for older adults, disability and social service information. Currently this directory contains information on not-for-profit public health and human service programs.

What are the eligibility requirements for the different programs/agencies?
Eligibility requirements for the different programs/agencies will vary from program to program. Individuals will need to discuss their eligibility requirements with the provider of choice. Connect to Care functions to provide access and assistance to the choices available.

Can I get help over the phone or in person?

How can I make the text larger or smaller?
At the top right corner of every page you'll find a "Text Size" control A A A A Select one of the four options. The screen will refresh in the selected text size. You can change the text size at any time during your visit to the site.

Why are search results found in Dallas County only?
We only provide services to Dallas County residents. If you live in a surrounding county and are looking for services provided by an ADRC in your area please read Other ADRCs in Texas.
